Street View-Sep 2013 Jefferson St NE Narrative(s): Title: Original Officer Narrative Narrative: On 03-19-2018 at 1033 hours, I, Officer Troy Vaughn responded to a personal injury incident involving a vehicle running into a house at the address of 4630 Jefferson St. NE. When I arrived, I spoke with the driver, Gregory Glaus, who stated that he was driving westbound on 46 1/2 Ave NE when his brakes went out causing him to jump over the retaining wall and into the house. He stated that it was not any ice or anything else that would cause him to run into the house besides the brakes on his vehicle. A passenger, Dorothy Brekke, was still stuck inside of the vehicle. Fire and public works freed Brekke from the vehicle and she was placed onto an ambulance truck along with the other passenger, Marjorie Langren. They were both transported to the hospital for precaution. I spoke with the reporting person, Linda Teigland, who stated that she didn't see anything but heard a loud bang which caused her to look and see that a vehicle crashed into the front of the house. Teigland stated that nobody in her home was injured and managed to get out of the house safely. Linda was in the house with her two grandsons and another family member, Diana Teigland. Officer Piehn took pictures of the incident with his Axon camera and uploaded them. The city's building inspector, Larry Pepin, responded to examine the home. Mr. Pepin stated that the vehicle was not holding the house up and gave the approval for JBS to tow the vehicle out of the house. JBS towed the vehicle out of the house and back to JBS tow lot. Allina's run number is 181146992. I gave each party a blue card with the case number on it. Glaus's car insurance was the General's Insurance (Policy# 71-MN2913186). A MN State Accident Report has been completed. A City of Columbia Heights Damage to City Property form has been completed. Driver stated his brakesfailed and he went through intersection, driving over the retaining wall, and striking the house at 4630 Jefferson Street. 01/20/2017 at 6:40 am: Property Damage Accident: Driver was westbound on 46 ½ Ave and slid into the intersection striking the guardrail. 11/30/2015 at 8:24 pm: Property Damage Accident: Driver was westbound on 46 ½ Ave and slid into the intersection striking the fence and partially going over the retaining wall. 11/17/2014 at 1:39 pm: Hit and Run Property Damage Accident:Suspect Vehicle struck and damaged sign, guardrail, shrubs, and chain link fence at 46 ½ and Jefferson. Suspect driver was not identified. 12/02/2013at 6:30 pm: Property Damage Accident: Driver was westbound on 46 ½ Avenue and lost control of the vehicle and struck bushes and handrail just east of Jefferson. No injuries. cc:Kathy Young, Assistant City Engineer MINNESOTA MANUALON UNIFORM TRAFFIC CONTROL DEVICES FEBRUARY 2015 The MUTCD guidelines for installingSTOP or YIELDsigns at intersectionsare listed below: Engineering judgment should be used to establish intersection control. The following factorsshould be considered: A.Vehicular, bicycle, and pedestrian traffic volumes on all approaches; B.Number and angle of approaches; C.Approach speeds; D.Sight distance available on each approach; and E.Reported crash experience. YIELD or STOP signs should beused at anintersection if one or more of the following conditions exist: A.An intersection of a less important road with a main road where application of the normal right-of-way rule would not be expected to provide reasonable compliance with the law; B.A streetentering a designated through highway or street; and/or C.An unsignalized intersection in a signalized area. In addition, the use of YIELD or STOP signs should beconsidered at the intersection of two minor streets or local roads where the intersection hasmore than three approaches and where one or more of the following conditions exist: A.The combined vehicular, bicycle, and pedestrian volume entering the intersection from all approaches averages more than 2,000 unitsper day. B.The ability to seeconflicting traffic on an approach is not sufficient to allow a road user to stop or yield in compliance with the normal right-of-way rule if such stopping or yielding is necessary; and/or C.Crash records indicate that five or more crashes that involve the failure to yield the right-of-way at the intersection under the normal right-of-way rule have been reported withina 3-year period, or that three or more such crashes have been reported withina 2-year period. YIELD or STOP signs should not be used for speed control.
