Re>zuires Prager Maintenance forlAtarranty Validation This Preventive Maintenance Service Agreement wfli be initiated, scheduled, administered, monitored and updated by Master Mechanical Inc. The Preventive Maintenance activities will be scheduled on a Quarterly basis, by our comprehensive equipment maintenance scheduling system, based upon seasonal reauirements. manufactures' recommendations, equipment location, application, type, run time and Master Mechanicais' technical experience. MMI will provide a professional Preventive Maintenance Plan as defined in Schedule 8 consisting of Testing, Inspecting, Cleaning, Aligning, Calibrating, Tightening, Adjusting, Lubricating and Analysis of the buildings mechanical system(s) comprised from equipment listed on Schedule A. Schedules which are attached hereto and made part of this Agreement, are in accordance with the terms and conditions set forth in th(s document. These services will be provided at the following address: o Cotumbta Weights Public Safety, 825 49st Avenue N.E., Columbia Weights, MN, 55421 This proposal is the proprietary property of Master Mechanical Inc. and is provided for Customer's use only. Master Mechanical Inc, guarantees the price stated in this Agreement for thirty (34) days from November 30. 2009.This proposal will become a binding Agreement only after acceptance by the Customer and approval by the Service Division Manager or the Service Sales Representative for Master Mechanical Inc. as evidenced by their signatures below. This Agreement sets forth ail the terms and conditions binding upon the parties hereto; and na person has the authority to make any claim, representation, promise or condition on behalf of Master Mechanical inc. which is not expressed herein. The initial term of this Agreement is 3 ears from the date of signafure and this Agreement shall continue in effect from year to year thereafter unless either party gives written notice to the other of intention not to renew thirty (3t}) days prior to the 3 ear anniversary date. MMI will notify the customer in advance of any price change specific to the agreement amount and f or labor rate prior to renewal on any given anniversary date. If results from the Preventive Maintenance indicate that repairs are required, a firm quotation will be submitted for Customers approval. All service work that is necessary will be at the reduced tabor rate of $91.00 I hour. Aii invoice statements will include an associated tructc charge of $45,00.
