User Notes: r nis document was produced by (1551678760} 7 ExxIBIT A OTHER PROVISIONS TO OWNER/CONTRACTOR AGREEMENT COLUMBIA HEIGHTS PUBLIC SAFETY CENTER COLUMBIA HEIGHTS, MINNESOTA Within five (5) working days after award of the Contract, this Contractor must submit to the Construction Manager, a list of materials, products, equipment, materials suppliers and subcontractors they will use to perform their portion of the project. 2. Within five (5) working days and prior to starting work on site, this Contractor must submit Insurance Certificates for the project in the amounts detailed in Specification Section 00810 -Supplemental and Special Conditions to General Conditions of the Contract for Construction. The Owner, Architect, and EDS Builders shall be named as additional insureds. 3. Within five (5} working days after award of the Contract, this Contractor shall submit a detailed schedule showing material deliveries, major work items, working periods and projected completion of the contracted portion of the work. The Contractor's schedule shall be in accordance with Schedule Section 00210, unless otherwise noted in this agreement. 4. Within five (5} working days after award of the Contract, this Contractor shall provide a typed schedule of values on AIA Document G703 - Schedule of Values, with breakdown of material and labor as directed by the Construction Manager, and in compliance with Article 5.4 of this Contract. 5. Within five (5) working days after award of the Contract, and prior to beginning work on-site, this Contractor must submit its safety program to the Construction Manager for review. 6. Prior to beginning work on-site, this Contractor shall submit to the Construction Manager's site office, material safety data sheets for all applicable materials to be used on-site. 7. The Contractor shall coordinate its safety program with that of the other Contractors on the site as specified in the Contract Documents including all Government regulatory agencies having jurisdiction on this project. 8. Inclusions and/or exclusions noted in the Contractor's proposal are not included unless specifically stated in this agreement. 9. Contract Sum stated in Article 4.1 includes the cost of all necessary or required insurance, permits, sales tax (if applicable), royalties, patents, etc. CITY OF COLUMBIA HEIGHTS -- OUTSIDE PLANT FIBER OPTIC CABLING- TO NEW PUBLIC SAFETY Bi.TILDING TECHNICAL SPECIFICATIONS l A SUMMARY The City of Columbia Heights (customer) is currently constructing a new Public Safety Building (PSB} that will house off ces and facilities far Police and Fire Departments. The new PSB is scheduled for completion in September of 2009. Current Police and Fire Departments are located within the existing City Hall Facility. Various systems will need to be linked betweea~ the new PSB and the existing City Hall. It is the intent of this RFP to solicit bids fora 36 strand singlemode Outside Plant Fiber Optic Backbone installed between the new PSB and the City Hall. In addition the customer would also require the pricing for various Alternates. A. BASE SID Provide and install a 35 strand singlemade, armored, filled outside plant (OSP) fiber optic backbone between the existing City Hall and t13e new Public Safety Building (PSB). Fiber cable to be installed within a new l .5" HDPE duct at a depth of 30"-36" bellow grade throughout entire route. Provide hand holes as indicated on drawing T 1.1 and coil 50' of spare fiber within each. Contactor will also provide a locate wire run in conjunction with the fiber throughout the entire route. This fiber optic backbone will run as a continuous segment throughout the entire outside plant portion without splices. Entry into the City Hall building demarc will be through existing conduit per Tl. l . Entry into the new PSB will be by new 4" conduit provided by the on site Electrical Contractor under the new PSB General Construction Contract. The canduit entry into the PSB will be directly into the demark/Phone-Server room 018. Termination will be on LC connectors within rack mount fiber patch panels, see detail drawing T1.2. In the City Hall building the fiber will enter into the demarc room 119 via existing conduit where it will be spliced and extended via inside building fiber OFNP cabling to the phone room 1 17 and the Server room 210, termination on LC connectors within wall mounted fiber optic patch panels see detail drawing T 1.2. The splice in the demarc room 119 of the City Hall will also require a few fiber strands to be spliced onto an existing. singlemode OSP cable that runs to the Public V4'orks building., see detail drawings TI.2 All fiber strands must be functional per the acceptance/testing criteria. Contractor will provide an as built document per specification. B. ALTERNATE l (12 strand OSP Fiber between new PSB and Library} Provide an added cost to install a 12 strand singiemode, armored, filled Outside Plant Fiber Optic Cable between the new PSB and the Library. This fiber can be installed within the same portion of HDPE duct and hand hales as the base bid fiber. Coil SO' of spare fiber within each hand hole in route. Entry into new PSB thro~gh new 4" conduit as provided. Terminate of all fibers in the new PSB in de arc/Server room 018 an LC connectors and rack mount fiber optic patch panel. Entry into the existing Library building via the existing hand hole and conduit into the Library boiler room. In library boiler room provide wall mount splice box and extend fiber via a new I2 strand singlemode OFNP Fiber to the phone/network room (approx distance is 75ft}. Terminate all fibers on LC connectors within a wall mount fiber optic patch panel. See drawings TLl and T1.2 C. ALTERNATE 2 (ZS-pair OSP Copper Cable between PSB and Library) Provide an added cast to install a 25-pair, copper, armored, filled C?utside Plant Cable between the new PSB and the Library. This capper cable can be installed within the same portion of HI7PE duct and hand holes as the base bid fiber. Coil SO' of spare cable within each hand hole in route. Entry into the new PSB as explained through new 4" conduit into the demarc/Server room 018. Terminate copper cabling on Entrance terminals with Gas Tube Lightning Arrestors and extend onto backboard via 25pr CAT.3 and 110 block. Band and ground cable accordingly. Entry into the Library through existing hand hole and conduit. Terminate copper cable in boiler room on Entrance Terminal with Gas Tube Lighting Arrestors, and extend copper onto the phonelserver room via a 25-pair CAT.3 CMP cable, approx 75', terminate in phone/server room on 110 block, Bond and Ground Cable accordingly. See drawings T1.1 and T1.2 D. ALTERNATE 3 (Spare Duct between PSB and Library} Provide an added cost to install a spare 1.5" NUPE duct adjacent to duct for base bid in the route between the new Public Safety Building and the Library. The spare duct wilt have a pull rope installed throughout the entire route and will enter into each building accordingly. Location of Existing Underground Utilities Coordinate locate meet with Gopher State One-Call for location of all public and private utilities registered within the specified route D. Restoration This contractor is responsible to restore all grounds and roadways disturbed from the associated fiber optic cable plant construction. Grounds and roadways must be restored to the original condition. Coordinate restoration with Constnzction Manager and Public Works as required. In addition this contractor will coordinate exact placementlelevation of each hand hole with the Construction Manger, and as required by permit. E. Permits This contractor is responsible to obtain and pay for all permits required from the City of Columbia Heights in placing this fiber optic cable plant.
