Dear Mr. Smith: WORK SCOPE /~~\~ 2088 } ~~/ The purpose of the Phase I ESAs are to assess the potential for environmental liabilities associated with the subject properties. We propose that the Phase I ESAs be conducted according to the following work scope: Historical Site Utility We will review aerial photographs, USGS topographic maps, Sanborn Fire Insurance maps. We will contact utility and regulatory contacts. In addition, we will interview current and past subject property owners, if available within the time frame of this study, regarding property usage. Site Reconnaissance We will conduct a site reconnaissance to document existing site conditions. Regulatory Agency File Review We will review in-house regulatory agency listings and request a file search on the subject property which will indicate if there has been a reported release or threatened release of a hazardous substance, pollutant or contaminant at or within one mile of the subject properties. Reporting We will issue a report summarizing our historical research, regulatory review and site reconnaissance. Conclusions and recommendations relative to liability and environmental concerns will be presented. Our opinions, conclusions and recommendations will be based solely upon the information and data collected as well as information provided by the client and/or property owners during the performance of the site assessments. COST ESTIMATE Phase I can be provided at a cost of $1380.00* * Phase I's do not include drilling, destructive testing, analysis or other means of evaluation outside of an on-site walk through. STANDARD OF CARE Phase I Environmental Site Assessment is conducted in accordance with currently accepted local environmental assessment practices. No other warranty is implied or intended. A CCF.P"T A NC'F This proposed work scope and associated cost estunate is based on our knowledge of the project site conditions. The bid submitted is valid 10 days from date, unless otherwise stated in writing. Payment is due upon completion of the job unless other arrangements are made. Interest of 1.5% will be charged per month on delinquent accounts. If payment is not received when due, AllPhase may take steps- necessary to enforce its rights under this agreement. The customer agrees to pay all costs incurred in collection with any delinquent account, including attorney's fees, whether a lawsuit is commenced or not. Your signature on this document acknowledges your acceptance of the proposed work scope and associated costs. Liability will be limited to $100,000.00. If additional Liability insurance is requested an additional cost will be incurred. Please return one copy of the signed proposal to AllPhase.
