D. Adopt Resolution 2005-02, Management Assessment for Public Housing Agencies
Motion: Move to Adopt Resolution 2005-02, a Resolution of the Columbia Heights
Housing & Redevelopment Authority confirming approval of the Management Assessment
for Public Housing Certification for 2005; and furthermore, to direct the Executive Director
to forward same to the U.S. Department of Housing and Urban Development.

BACKGROUND: At the October 18, 2004 Regular HRA Meeting, the Board reviewed and approved
the plans and specs for the second of eight phases to modernize all PVVN units. The remodeling will
include the same items as the 9th floor - new doors, cabinets, appliances, lighting, shelving, countertops,
et al. - plus new carpeting for the 8th and 9th floors. The Architect's Estimate for the work was $95,000.
Walker is assisting in the project by using their buying power to get the best prices on the appliances,
which will be installed by the Contractor, resulting in additional cost savings. The project was advertised
in the Focus, the Builder's Exchange of Minneapolis and St. Paul, McGraw-Hill Construction Data, Reed
Construction Data and the National Association of Minority Contractors' Plan Room, which resulted in 12
bids. Funding will come from the FY 2003 and FY 2004 HUD Capital Fund Program. Award and
signature of the contract would mean that all of the FY 2003 CFP Funds would be obligated prior to the
September 30, 2005 deadline. Once the contract is approved, a Notice to Proceed letter will be written
authorizing the work to begin. Finally, the bids are good for 45 days upon the bid opening on February
1 st, which necessitated this Special Meeting. Steve Erickson, the Project Architect, will be at the meeting
to answer any other questions. The 3 lowest bids are summarized in the next paragraph and the bid tab is
attached for review.
Below are the bids for the 8th floor rehab general contractor proposalslbids. The bids closed on February
1, 2005 at 2:00pm in the afternoon. There was a large quantity of general contractors who attended the
bid reading on that day. The general contractor that is low bid this time is also the same contractor that
completed the work on the 9th floor rehab.
Unicorp: Is the lowest bid in the amount of $79,200.00 for the 8th floor rehab of all 12 units, to include
the replacement of the hallway carpet on the 9th and 8th floor.
GA Construction: was the second lowest bid in the amount of $83,900.00 for the 8th floor rehab of all 12
units, to include the replacement of the hallway carpet on the 9th and 8th floor.
Crossroads Construction: was the third lowest bid in the amount of $87,900.00 for the 8th floor rehab of
all 12 units, to include the replacement of the hallway carpet on the 9th and 8th floor.
RECOMMENDATION: Staffrecommends the HRA approval of the bids and award the contract for
Phase 2 of the PVVN Apartment Modernization Project to UniCorp for and amount not to exceed
$79,200.00.
RECOMMENDED MOTION: Move to Approve the bids and award the contract for Phase 2 of
Parkview Villa North Apartment Modernization Project to UniCorp for an amount not to exceed
$79,200.00.

We have reviewed the bids received on February 1, 2005, for the renovation of the
8th Floor of Parkview Villa North. The low bid of $79,200 was submitted by
Unicorp. \X1e have reviewed the documents submitted by Unicorp and confIrmed
that Unicorp has no exceptions to their bid, that it incorporates all aspects of the
bid documents and they will honor their bid.
Based on our review and discussions with Unicorp, we recommend the City of
Columbia Heights enter into a contract with Unicorp for the renovation of the 8th
Floor of Parh.'View Villa North.

without. for the purpose of restricting competition. any consultation,
communication. or agreement with any other bidder or competitor
relating to (i) those prices, (ii) the intention to submit a bid. or (iii) the

exert improper influence to solicit or obtain contracts nor holds out
as being able to obtain any contracl(s) through improper influence.
"Improper influence" means any influence that induces or tends
to induce a PHNlHA employee or offICer to give consideration or to
act regarding a PHAlIHA contract on any basis other than the merits
of the matter.
(b) The bidder represents and certifies as part ofits bid that, except
for full-time bona fide employees wooong solely for the bidder, the

(a) The bidder's attention is called to the clause entitled Equal

and other eating areas, time clocks, locker rooms and other storage

principals or officers as a result of the contracl for which this proposal is submitted is attached hereto.
E. Previous Participation Certificate, Form HUD 2530, as provided for in the bid package is attached hereto and
furnished as a basis for the Owner to evaluate the Bidder to ascertain that the Bidder is a responsible contractor
possessing the potential ability to perform successfully under the terms and conditions of the proposed contract
giving consideration to such matters as contractor integrity. compliance with public policy, record of past
performance and technical resources.

H. The Bidder agrees, if awarded the Contract, to commence the work within 10 calendar days after the date of
execution of the construction contract.
1. [n submitting this bid, it is understood that the right is reserved by City of Columbia Heights to reject any and all
bids; and in the event the lowest responsible bid is too high to permit the award of a construction contract, and if a
reduction in cost less than one half (1/2) percent of the low bid and less than one half (112) the difference between
the low and next lowest bid would result in an acceptable award, and acceptable reductions in the work can be
identified and monetized by Owner and Low-Bidder, a deductive change order reflecting such reductions may be

without, for the purpose of restricting competition, any consultation,

(a) Definitions. As used in this provision:
"Bona fide employee" means a person, employed by a bidder

as being able to obtain any contract(s) through improper influence.

4. Organizational Conflicts of Interest Certification
The bidder certifies that to the best of its knowledge and belief and
except as otherwise disclosed, he or she does not have any
organizational conflict of interest which is defined as a situation in
which the nature of work to be performed under this proposed
contract and the bidder's organizational, financial, contractual, or
other interests may, without some restriction on future activities:

INSURANCE COVERAGE
On Nov. 26. 2002, President Bush signed into law the Terrorism
Risk Insurance Act of 2002 (the "ActU). The Act establishes a
short-term program under which the federal government will share
in the' payment of covered losses caused by certain acts of
international terrorism. We are providing you with this notice to
inform you of the key features of the Act, and to let you know what
effect, if any, the Act will have on your premium.
Under the Act, insurers are required to provide coverage for
certain losses caused by international acts of terrorism as defined
in the Act. The Act further provides that the federal government
will pay a share of such losses. Specifically, the federal
government will pay 90 percent of the amount of covered losses
caused by certain acts of terrorism, which is in excess of an
insurers statutorily established deductible for that year. The Act
also caps the amount of terrorism-related losses for which the
federal government or an insurer can be responsible at
$100 billion. provided that the insurer has met its deductible.
Please note that passage of the Act does not result in any change
in coverage under the attached policy or bond (or the policy or
bond being quoted). Please also note that no separate additional
premium charge has been made for the terrorism coverage
required by the Act. The premium charge that is allocable to such
coverage is inseparable from and imbedded in your overall
premium, and is no more than one percent of your premium.

BACKGROUND: As any of you can tell from entering the building here at Parkview Villa the 1 st
floor carpet, which is the heaviest trafficked area, is in need of replacing. We did have a contractor
come in the building and shampooed all North-building floors in October 2004. This process did not
seem to make a huge impact as the carpet is aged and worn and in need of replacement. In the office
area of the 1 st floor we have large buckles in the carpet caused from a leak the building incurred last
spring as the roof leaked down a wall and pooled in the 1 st floor office. At the time the water was
finally cleaned up and dried the carpet gave way and a l-Y2 -2 inch buckle formed.
It is to be believed first that the appearance of the carpet upon entering the building makes the
building less desirable to live in and the buckle in the office areas pose as a trip hazard. Therefore
three (3) bids were obtained to replace the carpet on the 1 st floor only of Park view Villa from funds
that we have left in the budget from 2003' s capital improvement funds.
Jerry's Floor Store: was the lowest bid in the amount of $8,980.79 for the replacement of the 1 st floor
carpet, including a higher density carpet at the front entry door and to continue to the mailboxes on
the 1 st floor. These are going to be carpet tiles therefore in the event there are some that need to be
replaced we can do so in sections or pieces rather that the entire 1 st floor. This will make that section
only a different color of carpet then the rest of the floor.
United Floor Covering Service: is the second lowest bid in the amount of$10,296.00 for all of the
same details listed in the above bid.
St. Paul Linoleum: is the third lowest bid in the amount of$12,465.00 for all of the same details listed
in the above bid.
RECOMMENDATION: Staff recommends approval of the contractor Jerry's Floor Store, which is
the lowest bid for the 1 st floor carpet replacement.
RECOMMENDED MOTION: Move to approve the bid from Jerry's Floor Store to complete the 1 st
floor carpet replacement.

BACKGROUND: A Board Resolution is required on an annual basis for approval of the
Management Assessment for Public Housing Agencies. Attached is HRA Resolution 2005-02, along
with the related form to be submitted to HUD for compliance purposes by electronic submission
through the REAC system.
The MASS is a Management Operations Overview and includes the vacancy rate and unit turn around
time, Modernization, Rent Collected, Work Orders and Annual Inspection of units, Financial
Management, Resident services and Community Building and Security.
RECOMMENDATION: Staff recommends Adoption of Resolution 2005-02, a Resolution
approving the Management Assessment for Public Housing Agencies Certification for 2005.

RESOLUTION OF THE COLUMBIA HEIGHTS HOUSING &
REDEVELOPMENT AUTHORITY CONFIRMING APPROVAL OF THE
MANAGEMENT ASSESSMENT FOR PUBLIC HOUSING AGENCIES
CERTIFICATION FOR 2005.
WHEREAS, The Department of the Housing and Urban Development requires
that Public Housing Agencies submit the Management Assessment for Public Housing
Certification for 2005; and
WHEREAS, such Certification also requires the formal approval of the Board of
Commissioners;
NOW THEREFORE BE IT RESOLVED by the Board of Commissioners of the
Columbia Heights Housing & Redevelopment Authority that the attached the
Management Assessment for Public Housing Certification for 2005 and the necessary
attachments are hereby approved.
