The following is the agenda for the regular meeting of the Columbia Heights Housing &
Redevelopment Authority (HRA) to be held at 6:30 PM on Tuesday, November 20, 2001, in the
Parkview Villa Community Room B, 965 40th Avenue N.E., Columbia Heights, Minnesota.

CALL TO ORDER - The Regular Meeting of the Columbia Heights Housing & Redevelopment Authority (HRA)
was called to order by Chair, Marlaine Szurek, at 6:01 p.m., September 18, 2001, in the Parkview Villa Community
Room B, 965 40th Avenue NE, Columbia Heights, Minnesota.

Nawrocla' felt the HRA meeting should have been held after the EDA meeting and that all commissioners should
have been notified of the 6:00p. m. start time.
CONSENT AGENDA
Approval of Minutes of Meeting of August 21, 2001.
MOTION by Nawrocki, second by Williams, to adopt the minutes of August 21, 2001 regular meeting as presented
in writing. All ayes. Motion Carried.
PUBLIC HEARING
HUD Agency Plan and Approval of Resolution 2001-03
Schumacher reported that the Quality Housjng and Work Responsibility ~4ct of 1998 requires all Housing
Authorities to prepare a one year Agency Plan. Staff has completed the draft copy for public review, the required
forty-five day review period for public input has been satisfied, the Parkview Villa Resident Advisory Board, and
Anoka County has reviewed the plan. The entire document will be electronically transmitted through the HUD
REA C system. Staff recommended the Board open the Public Hearing for comments, adopt Resolution 2001-03 a
resolution of the Columbia Heights HR,4 confirming approval of the Public Housing ~4gency Certifications of
Compliance, with the PH,4 Plans and related regulations.
Nawrocki questioned why the PHA Plan was brought before the HRA and not the EDA. Schumacher clarified that
HUD recognizes the HR21, not the EDA with anything related to Public Housing Nawrocla' requested staff provide
him a copy of last years HR,4 meeting minutes were the PHA Plan was addressed..
Chair Marlaine Szurek opened the Public Hearing at 6:10 p. m.
Resident Renie Sufka, Unit 104, asked why the cupboard replacement is scheduled for two units per year and if
there isn't enough funds available why couldn't the antenna fund that was designated for Parkview Villa be used.
Szurek explained that the EDA would like to replace more than two units per year, but with the budget cuts for next
year this isn 't feasible. Ruettimann stated the funds from the antenna have never been designated to Parkview
Villa..
MOTION by Williams, second by Peterson, to close the Public Hearing for the review of the draft Housing and
Redevelopment Authority Annual PHA Plan at 6:20 p.m. All ayes. Motion Carried.
MOTION by Williams, second by Peterson, to waive the reading of Resolution 2001-03, there being ample copies
available to the public. All ayes. Motion Carried.

MOTION by Williams, second by Peterson, to adopt Resolution 2001-03 of the Columbia Heights Housing and
Redevelopment Authority (HRA) confu'ming approval of the Public Housing Agency certifications of compliance
with the PHA plans and related regulations. Upon Vote: Wyckoff- Aye, Williams- Aye, Peterson- Aye,
Nawrocki-Nay. Motion Carried.
Transfer of Home Mortgage Loans
Schumacher stated that in 1979, there were two Home Mortgage Loans made by the HRA. The Chartraw/Uecker
loan was closed on June 1, 1979for 30years, monthly payments of $381.34 and has a balance remaining of
$19,725.57. The Neilsen loan was also for 30years, monthly payments of $318. 70 and has a remaining balance of
$18,064.33. Interest on both loans is 5%. In May, the EDA Board directed staff to sell the two loans to a mortgage
company. Staff received an offer from Community Reinvestment Fund (CRF) for $32,82 4.31for both loans. The
ED,4 Board approved the sale at their July meeting, which was later determined the loan was taken out originally
by the HRA. The sale of the two loans needs to be approved by the HRA with proceeds to be placed into the
Business Revolving Loan Fund.
MOTION by Peterson, second by Williams, to approve the sale of the Chartraw/Uecker and Nielsen Home
Mortgage Loans to Community Reinvestment Fund for approximately $32,824.31 with the proceeds from the sale
placed in the Business Revolving Loan Fund; and furthermore, to authorize the President and Executive Director to
enter into an agreement for the same. All ayes. Motion Carried.
Warriek Sheffield Loan Payoff
Schumacher stated on May 24, 1995, the HIM issued a Sheffield Home Improvement Loan to John S. and Mary C.
Warrick, 4636 Fillmore Street NE, for roofing and siding their home. This is the last Sheffield Loan staff receives
monthly payments on. Staff offered the }garrick's a 5% discount for early payoff of the remaining balance of
$1,992.71. On Wednesday, September 12a, Mr. Warrick met with staff to discuss their options. As a result of the
meeting, Warrick is prepared to make a final payment in the amount of $1,811.89, provided this action is approved
by the HIM.
MOTION by Peterson, seconded by Williams, to approve $1,811.89 as the payoff amount for the Sheffield Home
Improvement Loan Promissory Note, dated May 25, 1995, from John S. and Mary C. Warrick, to be deposited into
the Business Revolving Loan Fund; and furthermore, to authorize the President and Executive Director to enter into
an agreement for the same. All ayes. Motion Carried.

BACKGROUND: Staff has prepared a draft copy of the Columbia Heights Agencies Annual Plan for
2002. HUD requires a forty-five day review period to allow for Public input. To meet this requirement,
the Columbia Heights HRA could conduct a Public Hearing on January 15, 2002. At this Hearing the
HRA Board would make final modifications to the plan and authorize staffto submit the final document
to HUD. The resident advisory board as well as Anoka County will receive copies for their review and
comment. The entire document has been generated by electronic transmission and downloaded through
the HUD Web page REAC system. This will allow the HRA easy access to the plan and simplify the
process in its annual update. A full copy of the draft plan with attachments and supporting documents
will be available for review at the Columbia Heights HRA office, 590 40t~ Avenue NE, Columbia
Heights, Minnesota.
RECOMMENDATION: Staff is recommending that the HRA Board set a Public Hearing date of
January 15, 2002 at 6:30 p.m. at Parkview Villa for the purpose of receiving public input on the 2002
draft Agency Plan.

RESOLUTION OF THE COLUMBIA HEIGHTS HOUSING & REDEVELOPMENT
AUTHORITY (HRA) CONFIRMING APPROVAL OF THE 2002 PUBLIC HOUSING
AGENCY CERTIFICATIONS OF COMPLIANCE, WITH THE PHA PLANS AND
RELATED REGULATIONS.
WHEREAS, The Department of the Housing and Urban Development has requested that
Public Housing Agencies submit a Certification in regard to Public Housing Agency Compliance
with the PHA Plans and Related Regulations; and
WHEREAS, such Certification also requires the formal approval of the Board of
Commissioners;
NOW THEREFORE BE IT RESOLVED by the Board of Commissioners of the
Columbia Heights Housing & Redevelopment Authority that the attached (Appendix AA~)
Public Housing Agency Certifications of Compliance with the PHA Plans and Related
Regulations is hereby approved.

and Related Regulations
Board Resolution to Accompany the PHA Plan
Acting on behalf of the Board of Commissioners of the Public Homing Agency (PHA) listed
below, as its Chairman or other authorized PHA official if there is no Board of Commissioners,
I approve the submission of the_' Annual P 1 an for PHA fiscal year beginning
2002 , hereinafter referred to as the Plan of which this document is a part and make the
following certifications and agreements with the Department of Housing Development (HUD) in
connection with the submission of the Plan and implementation thereof:
1. The Plan is consistent with the applicable comprehensive housing affordability strategy (or any plan
incorporating such strategy) for the jurisdiction in which the PHA is located.

3. The PHA has established a Resident Adviso~ Board or Boards, the membership of which represents
the residents assisted by the PHA, consulted with this Board or Boards in developing the Plan, and
considered the recommendations of the Board or Boards (24 CFR 903.13). The PHA has included in ~e
Plan submission a copy of the reco~endations made by the Resident Adviso~ Board or Boards and a
description of the ma~er in which the Plan addresses these recommendations.
4. The PHA made the proposed Plan and all infomation relevant to the public hearing available for public
inspection at least 45 days before the hearing, published a notice that a hearing would be held and
conducted a hearing to discuss the Plan and invited public comment.
5. The PHA will c~ out the Plan ~ confo~i~ wi~ Title VI of ~e Civil Rights Act of 1964, the Fair
Housing Act, section 504 of the Rehabilitation Act of 1973, and title II of the Americans with Disabilities
Act of 1990.
6. ~e PHA will affimatively ~er hir housing by examining their programs or proposed programs,
identify ~y impediments to hir housing choice within those programs, address those impediments in a
reasonable hshion in view of the resources available and work wi~ local j~isdictions to implement any
of the jurisdiction's initiatives to affimatively h~her hir housing that require the PHA's involvement
and maintain records reflecting these analyses and actions.
7. For PHA Plan ~at includes a policy for site based waiting lists:
The PHA regul~ly submits required data to HUD's MTCS in an accurate, complete and timely
m~er (as specified in PIH Notice 99-2);
- The system of site-based waiting lists provides for ~11 disclosure to each applicant in the selection
of the development in which to reside, including basic infomation about available sites; and an
estimate of the period of time ~e applic~t would likely have to wait to be admitted to units of
different sizes and ~pes at each site;
- Adoption of site-based waiting list would not violate any cou~ order or settlement agreement or be
inconsistent with a pending complaint brought by HUD;
- The PHA shall take reasonable measures to assure that such waiting list is consistent with
affi~atively h~hefing hir housing;

8. The PHA will comply with the prohibitions against discrimination on the basis of age pursuant to the
Age Discrimination Act of 1975.
9. The PHA will comply with the Architectural Barriers Act of 1968 and 24 CFR Part 41, Policies and
Procedures for the Enforcement of Standards and Requirements for Accessibility by the Physically
Handicapped.
10. The PHA will comply with the requirements of section 3 of the Housing and Urban Development Act of
1968, Employment Opportunities for Low- or Very-Low Income Persons, and with its implementing
regulation at 24 CFR Part 135.
11. The PHA has submitted with the Plan a certification with regard to a drug free workplace required by 24
CFR Part 24, Subpart F.
12. The PHA has submitted with the Plan a certification with regard to compliance with restrictions on
lobbying required by 24 CFR Part 87, together with disclosure forms if required by this Part, and with
restrictions on payments to influence Federal Transactions, in accordance with the Byrd Amendment
and implementing regulations at 49 CFR Part 24.
13. For PHA Plan that includes a PHDEP Plan as specified in 24 CFR 761.21: The PHDEP Plan is consistent
with and conforms to the "Plan Requirements" and "Grantee Performance Requirements" as specified in
24 CFR 761.21 and 761.23 respectively and the PHA will maintain and have available for
review/inspection (at all times), records or documentation of the following:
· Baseline law enforcement services for public housing developments assisted under the PHDEP
plan;
· Consortium agreement/s between the PHAs participating in the consortium and a copy of the
payment agreement between the consortium and HUD (applicable only to PHAs participating in a
consortium as specified under 24 CFR 761.15);
· Partnership agreements (indicating specific leveraged support) with agencies/organizations
providing funding, services or other in-kind resources for PHDEP-funded activities;
· Coordination with other law enforcement efforts;
· Written agreement(s) with local law enforcement agencies (receiving any PHDEP funds); and
· All crime statistics and other relevant data (including Part I and specified Part II crimes) that
establish need for the public housing sites assisted under the PHDEP Plan.
14. The PHA will comply with acquisition and relocation requirements of the Uniform Relocation
Assistance and Real Property Acquisition Policies Act of 1970 and implementing regulations at 49 CFR
Part 24 as applicable.
15. The PHA will take appropriate affirmative action to award contracts to minority and women's business
enterprises under 24 CFR 5.105(a).
16. The PHA will provide HUD or the responsible entity any documentation that the Department needs to
carry out its review under the National Environmental Policy Act and other related authorities in
accordance with 24 CFR Part 58.
17. With respect to public housing the PHA will comply with Davis-Bacon or HUD determined wage rate
requirements under section 12 of the United States Housing Act of 1937 and the Contract Work Hours
and Safety Standards Act.
18. The PHA will keep records in accordance with 24 CFR 85.20 and facilitate an effective audit to determine
compliance with program requirements.
19. The PHA will comply with the Lead-Based Paint Poisoning Prevention Act and 24 CFR Part 35.
20. The PHA will comply with the policies, guidelines, and requirements of OMB Circular No. A-87 (Cost
Principles for State, Local and Indian Tribal Governments) and 24 CFR Part 85 (Administrative
Requirements for Grants and Cooperative Agreements to State, Local and Federally Recognized Indian
Tribal Governments.).
21. The PHA will undertake only activities and programs covered by the Plan in a manner consistent with
its Plan and will utilize covered grant funds only for activities that are approvable under the regulations
and included in its Plan.

All attachments to the Plan have been and will continue to be available at all times and all locations that
the PHA Plan is available for public inspection. All required supporting documents have been made
available for public inspection along with the Plan and attachments at the primary business office of the
PHA and at all other times and locations identified by the PHA in its PHA Plan and will continue to be
made available at least at the primary business office of the PHA.

Acting on behalf of the above named Applicant as its Authorized Official, I make the following certifications and agreements to
the Department of Housing and Urban Development (HUD) regarding the sites listed below:
I certify that the above named Applicant will or will continue
to provide a drug-free workplace by:
a. Publishing a statement notifying employees that the un-
lawful manufacture, distribution, dispensing, possession, or use
of a controlled substance is prohibited in the Applicant's work-
place and specifying the actions that will be taken against
employees for violation of such prohibition.
b. Establishing an on-going drug-free awareness program to
inform employees ---
(1) The dangers of drug abuse in the workplace;
(2) The Applicant's policy of maintaining a drug-free
workplace;
(3) Any available drug counseling, rehabilitation, and
employee assistance programs; and
(4) The penalties that may be imposed upon employees
for drug abuse violations occurring in the workplace.
c. Making it a requirement that each employee to be engaged
in the performance of the grant be given a copy of the statement
required by paragraph a.;
d. Notifying the employee in the statement required by para-
graph a. that, as a condition of employment under the grant, the
employee will ---
(1) Abide by the terms of the statement; and
(2) Notify the employer in writing of his or her convic-
tion for a violation of a criminal drug statute occurring in the
workplace no later than five calendar days after such conviction;
e. Notifying the agency in writing, within ten calendar days
after receiving notice under subparagraph d.(2) from an em-
ployee or otherwise receiving actual notice of such conviction.
Employers of convicted employees must provide notice, includ-
ing position title, to every grant officer or other designee on
whose grant activity the convicted employee was working,
unless the Federalagency has designated a central point for the
receipt of such notices. Notice shall include the identification
number(s) of each affected grant;
f. Taking one of the following actions, within 30 calendar
days of receiving notice under subparagraph d.(2), with respect
to any employee who is so convicted ---
(1) Taking appropriate personnel action against such an
employee, up to and including termination, consistent with the
requirements of the Rehabilitation Act of 1973, as amended; or
(2) Requiring such employee to participate satisfacto-
rily in a drug abuse assistance or rehabilitation program ap-
proved for such purposes by a Federal, State, or local health, law
enforcement, or other appropriate agency;
g. Making a good faith effort to continue to maintain a drug-
free workplace through implementation of paragraphs a. thru f.

ii. Executive Summary
The Agency Plan for the Columbia Heights Housing and Redevelopment
Authority describes the programs, policies, and practices that were implemented in
FY 2000. To date, new security card procedures, lease revisions for lead base
paint and one strike revision, and policies for tracking crime and crime-related
problems and screening applicants criminal history have been enacted by the
Board. The Columbia Heights Housing and Redevelopment Authority is "on the
right track" in meeting the varied needs of residents and will continue to stress
sound property management and maintenance in the FY 2002 Agency Plan.
