OBJECTIVE OF PROGRAM IV. To provide services that will significantly reduce the potential of specific pest problems that commonly occur within residences and to provide such services in a prompt, courteous manner. I1. PESTS AND AREAS COVERED V. A. The elimination and prevention of: rats, mice, roaches*, silverfish, ants*, crickets, waterbugs, spiders and sowbugs. B. Common areas of the building(s) as well as all resident units are protected under terms of this agreement. VI. C. *SPECIAL PROGRAM FOR COCKROACHES AND (the fete) PHARAOH ANTS: Because these two pests are carried into the buildings, no preventative work can stop their invasion. Should either of these pests be brought into the building(s), additional costs may occur. Up to N/A units per N/A can be treated for roaches or pharaoh ants with no additional charge. Should more than the allotted number of units require treatment the additional units will be billed at the rate of SN/A per unit. VII. THERE ARE NO LIMITATIONS ON THE NUMBER OF UNITS TREATED FOR THE OTHER PESTS LISTED IN ITEM IIA. D. For the first N/A months of this agreement there will be a charge of SN/A per unit. II1. SCOPE OF OPERATIONS A. Twelve scheduled services will be performed over a twelve month period. On these services we will inspect and treat common areas such as: entryways, underground parking, laundry rooms, boiler rooms and storage rooms. B. Also on scheduled service dates we will address the seasonal needs of the property. Examples: Barrier treatments around the exterior of the buildings to stop foraging insects such as ants in the summer or to help stop invasions of insects such as crickets and boxelder bugs in the fall. LOG BOOK/RECORD KEEPING/INSPECTION REPORTS A. We will retain records of all work performed on the property throughout the year. B. An annual inspection report of the property will be made to pinpoint areas of concern in regards to pest prevention. SPECIAL REQUEST SERVlCESIRESIDENT COMPLAINTS All requests for service within the residents' units will be responded to promptly. CUSTOMER COOPERATION Periodically suggestions will be made to help in the overall scope of pest prevention. These requests will be made in the best interests of your property. Please regard this information as important and take the appropriate action to correct these concerns. Failure to cooperate with the correction of these conditions is proper cause for ADAM'S to void all warranties. COMMENTS OR SPECIAL INSTRUCTIONS The initial charqe splits the cost of rodent .qear and baitinq stations that will be placed in and around your property. After completion of the first year you own the equipment. VIII. TERMS & CONDITIONS Investment for ADAM'S "Prevention Plus" as outlined: Initial charge: $50.00, payable as invoiced. Maintenance Fee: $750.00, per year. Payable: $62.50, per month. 1) Terms are net 30. 2) If payment is not received before next billing date, a service charge of 1.5% of the unpaid balance is added to the amount due. This is an annual percentage rate of 18%. 3) It is your responsibility to notify us in writing in case of change of ownership, name or management change. 4) Prices do not include any applicable taxes. 5) If there is ever a need for ADAM'S to forward your account to a collection agency, you agree to pay for all collection charges incurred. This agreement shale be for an initial period of ( _'~ months.
