BEING A RESOLUTION AUTHORIZING PARTICIPATION IN THE
LEAGUE OF MINNESOTA CITIES INSURANCE TRUST
SELF-FUNDED DENTAL PLAN
BE IT HEREBY RESOLVED that the City Council of the City of Columbia
Heights authorizes the City Manager to provide for the City's participation in
the League of Minnesota Cities Insurance Trust Self-Funded Dental Plan,
effective May 1, 1986.
BE IT FURTHER RESOLVED that the City agrees to remit the monthly premium
from May 1, 1986, to July 1, 1987, based on the funding rates listed below.
Guaranteed Monthly Funding Factors - Dental Coverage:
Single ........ $11.23
Family ....... $40.10
