(ProSource) is pleased to present this scope of work and cost estimate to conduct a Phase I Enviromnental Site Assessment (ESA) and pre- demolition survey for the Burger King Restaurm~t (Site) located at 3939 Central Avenue Northeast in Columbia Heights, Mi~mesota. In addition, you requested that ProSource provide general project management until the building is demolished, thereby allowing for redevelopment of the Site as a residential property. The scope of work m~d cost estimate to conduct this work are provided in the following sections. Scope of Work Phase I ESA The ptapose of conducting the Phase I ESA is to meet properly transfer guidelines as set forth by the Mim~esota Pollution Control Agency (MPCA), to determine if a release or threatened release of a hazardous substance, pollutant, or contmninant exists or has occurred trader the meaning of the Mim~esota Environmental Response and Liability Act (MERLA). In addition, the Phase I ESA will assess the potential for enviromnental impact to this Site fi'om other nearby facilities also covered under MERLA. The Phase I ESA will be conducted following guidelines established by the American Society for Testing and Materials (ASTM), "Standard Practice for Environmental Site Assessments: Phase I Enviromnental Site Assessment Process, St~dard E1527-00". In Prog~ (VIC) Progmn Fact Sheet ~8 "Phase I Inwstigation" will be utilized as a guide for conducting the work. The work to be provided by ProSot~ce as pa~ of the Phase I ESA includes the following: Site Location - Describe the location of the Site by street address, town, ship and/or municipality, county, and United States Geological Smwey (USGS) Quadrants to the nearest 1/4 section. History of Ownership - Provide the histories of ownership for the Site over the past 50 years or as far back as there are records, including the name of each ow2~er and the approximate year(s) of ownership. Environmental · Right of Way * Engineering Scope of Work and Cost Estimate Burger Irdng Restaurant Page 2 History of Operations - For each owner/operator of the Site, detem~ine what activities they engaged in. In particular, determine operation processes, raw products used, types and composition of any waste streams, disposal methods, environmental permits and solid or hazardous waste streams including manifests, treatment, storage, disposal, spills or enforcement actions. Site Reco~maissance - Conduct a walkover of the Site to detenrfine the presence of items including, but not limited to under grotmd or above ground storage tanks (USTs or ASTs), refuse disposal, wells, septic systems, smnps, pits, ponds, lagoons, catch basins, retention ponds, areas of surface soil staining, stressed vegetation, or any other indication of a potential contamination source. Adjacent properties will also be evaluated to determine if they are potential sources of contamination. Site Maps - Provide historic and current site maps which designate the locations of all pertinent current or foriner features, structures and potential sources of contamination. · Database File Search - Conduct a database file search of all applicable State and Federal regulatory agencies to determine the presence of known contaminated properties near the Site. Information Requests - h~quire via letter or telephone inte~wiew with local officials regarding their knowledge of the Site. Persons to be contacted may include, but are not limited to the local engineehng and/or public works department, planning and development persounel fi:om the City, County or other development authorities, zoning and assessment persom~el from the City or County, the county solid waste officer, the county hazardous waste officer and/or the local fire department. · Aerial Photo~aphs - Review copies of aerial photographs, or reasonable facsimiles, to check for altered land use, previous storage, etc. · Sanborn Maps - Review copies oflfistoric Sanborn fire insurance maps, or reasonable facsimiles, to check for altered land use, previous storage, etc. · City Directories - Review historic city directories which have been published to determine the history of occupancy. All information gathered during the Phase I ESA will be presented in a report. In addition to the findings of the Phase I ESA, the report will also identify recognized enviromnental concerns (RECs) for the Site and will provide recmmnendations for any additional actions (if necessary). October 1, 2004 Scope of Work and Cost Estimate Burger King Restaurant Page 3 Pre-Demolition Survey The purpose of the pre-demolition survey is to identify and quantify regulated and/or hazardous materials which will require proper removal/abatement prior to building demolition. In addition, ProSource will define and quantify suspect asbestos containing material (ACM) and lead-based paint that may impact building demolition. Regulated/Hazardous Waste Inventory A regulated/hazardous waste inventory will be conducted to identify and quantify hazardous or regulated materials prior to building demolition. These materials may include, but are not limited to mercury, polychlorinated biphenyls (PCBs), chlorofluorocarbons (CFCs), fluorescent light bulbs, chelrdcals, flalmnable liquids, etc. All materials should be recycled and/or disposed of by a licensed contractor in accordance with all Local, State and/or Federal laws and regulations. Asbestos Samples of suspect ACM types will be collected and sealed in polyethylene bags. All materials will be wetted prior to sampling to minimize fiber/dust release to the enviromnent. For larger quantities of suspect ACM, at least three, but tip to seven random samples of a homogeneous material will be collected as a sanlple set to represent the material type. Homogeneous materials (i.e. thermal system insulation, ceiling spray, tile, etc.) are defined as those which appear to be the same on the basis of apparent age, color, size and textm'e. It should be noted that destructive sampling (i.e. puncturing walls) will be necessary to complete this smwey. A chain-of-custody will be initiated and kept with the samples until they are submitted to a qualified laboratory for analysis. Samples will be analyzed for asbestos content in accordance with Environmental Protection Agency (EPA) Method 600R-93/166 and will be examined using polarized light microscopy with dispersion staining teclmiques. Fibrous components will be identified and an area percentage of asbestos present will be determined by visual estimation. When applicable, the laboratory will use a test till positive scheme (meaning if the first sample of a set is found to contain asbestos, the remainder of the sub-set samples will be assumed to be positive and no further analysis will be conducted). This will serve to minimize analytical costs and may result in fewer samples being analyzed. Prior to building demolition, all ACM should be abated by a licensed contractor in accordance with all Local, State and/or Federal laws and .... ~*~^~ * C~umu~uS. Lead-Based Paint ProSource will identify and sample homogeneous paint material. Homogeneous paints are defined as areas of similar paint history such as color, location and texture. As with asbestos, paint samples will be placed into sealed polyethylene bags and a chain-of-custody will be initiated and kept with the samples until they are submitted to a qualified laboratory for analysis. Samples will be analyzed for lead in paint utilizing methods based on cmTent EPA protocol SW-846, Method 7420 and ASTM D-3335-85A. ProSource Teclmologies, Inc. October 1, 2004 Scope of Work and Cost Estimate Burger King Restaurant Page 4 The MPCA defines lead-based paint as a coating that contains lead a concentration of 5,000 parts-per- million (ppm) or greater. Homogeneous paint materials that contain less than 5,000 ppm (0.5% by weight) may be treated as non lead-based paint and will not require removal. Lead-based paint that is intact to its substa'ate does not require removal and may be disposed of in a demolition landfill. Substrates with lead-based paint coatings may not be crushed for recycling pm-poses unless the lead- based paint is removed prior to building demolition. Re£orting Upon completion of the pre-demolition survey, a report will be prepared. The report will include a discussion of the methods used for sampling and present the results for the asbestos and lead-based paint sampling and analysis. In addition, an itemized list of all materials identified during the regulated/hazardous waste inventory will be included in the report. Furthenr~ore, recommendations regarding the building will also be made (i.e. no further action, additional sampling, removal/abatement, etc.). Project Management Once the Phase I ESA and the pre-demolition survey have been completed, the City has requested that ProSource provide project management services to until the building is demolished, thereby allowing for redevelopment activities to begin. The primary tasks will entail preparing demolition specifications, procm-ing a demolition contractor and providing the necessary coordination/oversight required until the building and applicable structures have been removed. Assumptions No attempt will be made to perfo~Tn a compliance audit involving regulatory penxfit review for any on-site operations or adjacent areas of concern identified during the Phase I ESA, or for any planned use of the Site. The names and contact information (i.e. telephone number, e-mail adch'esses, etc.) for key persons with knowledge and/or access to the Site will be provided in a thnely fashion. Furthermore, it is assumed that all infonrtation compiled fi'om these persons (other than the cun'ent Property owner) can be obtained via telephone, fax and/or e-mail and conducting on-site IilL~I ¥1GW~ Will not uc necessary. We will have complete access to the Site and all related structures during reconnaissance and inspection activities. Furthermore, we are assuming that the building will be vacant at the time the pre-demolition survey will be conducted. ProSource Teclmologies, Inc. October 1, 2004 Scoge of Work m~d Cost Estimate Burger King Restaurant Page 5 Cost Estimate Based on the above scope of work, our lmnp smri cost to conduct the Phase I ESA is $1,800. The estimated time and materials costs to conduct the pre-demolition survey provide project management services ale $2 459 and $3,213,. respectively. These costs include all labor and expenses and are based on the following considerations: A total of 40 asbestos samples and 15 lead samples will be collected m~d m~alyzed by the laboratory as part of the pre-demolition survey. However, the City will only be billed for the actual number of samples analyzed by the laboratory. The cost estimate for project management assumes that 40 hours will be required, with $125 allotted for expenses. However, the City will only be billed for the actual muriber of hours charged to the project. Please note that our standard markup for subcontractors (i.e. laboratory) is 15%. If you would like ProSource to handle contracting with the demolition contractor, we would happy to do so at a reduced markup of 5%. If you would prefer that the City admi~fister contracting with the demolition contractor, we will see that invoices are forwarded directly to you for payment. This cost estimate applies to the scope of work, as described above. In the event that a REC is identified during the Phase I ESA and needs to be addressed (i.e. removal ofa UST, conducting a Phase II ESA, etc.), ProSource will prepare a separate scope of work and cost estimate. As necessary, invoices will be submitted to the City on a monthly basis. However, the lump sum cost for the Phase I ESA will not be submitted to the City tmtil the final report has been completed. Closin~ We appreciate the opportmfity to submit this scope of work and cost estimate. It is our understanding that the City would like the building demolished by mid to late November of 2004. As such, we can proceed conducting the Phase I ESA immediately, followed by the completion of the pre-demolition survey.
