HomeMy WebLinkAboutContract 1619 CITY OF COLUMBIA HEIGHTS AS GRANTEE FOR COLUMBIA HEIGHTS AFTER-SCHOOL ENRICHMENT PROGRAM CONTRACT FOR SERVICES IT IS HEREBY AGREED THAT SCHOOL DISTRICT #13 COMMUNITY EDUCATION DEPARTMENT (hereinafter called "CONTRACTOR") will furnish services as herein stated for the COLUMBIA Ir[EIGHTS AFTER-SCHOOL ENRICHMENT PROGRAM (hereinafter called "CHASE"). This agreement shall take effect upon its execution by both parties and shall remain in effect until June 1, 2001 at which time it shall be automatically terminated. This agreement is subject to termination by either party by notice in wri!ting to the other. SERVICES TO BE PERFORMED UNDER TERMS OF THIS AGREEMENT Provide the Middle School Tutoring program for youth in grades 6-8th will be held from April 1, 2001 through June 1, 2001. The Middle School Tutoring program will be located at Central Middle School. The CONTRACTOR shall provide all instructors, supplies and equipment for the program. The CONTRACTOR shall provide publicity and process the registrations for the Middle School Tutoring program. All other services to be performed by CHASE and the CONTRACTOR in connection with the Middle School Tutoring program are specified in the attached CHASE program letter of understanding, and the mini-grant proposal submitted by the CONTRACTOR, which are incorporated into this document by reference. CONSIDERATION TO BE PAID FOR SERVICES SHALL BE: The CONTRACTOR shall be paid a maximum of $1056.17 for the provision of the Middle School Tutoring program (See attachment for breakdown distribution). The CITY OF COLUMBIA HEIGHTS (hereinafter called "CITY") shall act as the fiscal agent for the disbursement of CHASE funds. Payment shall be issued by check approximately (3) three weeks after the completion of the program or if other arrangements have been made for disbursement. AGREED THIS 31ST DAY OF September, 2000. It is agreed that said services are to be rendered as an independent contractor and not as an employee of CHASE or the CITY. The CONTRACTOR understands that the performance of services as herein stated may involve some risk of injury or illness to the CONTRACTOR and to the participants of the Middle School Tutoring program. CHASE and the CITY will not be responsible for any injury or illness suffered by the CONTRACTOR, the CONTRACTOR's employees, the CONTRACTOR'S agents or the participants of the Middle School tutoring program during the time the CONTRACTOR performs the services herein stated. Furthermore, the CONTRACTOR agrees to defend, indemnify and hold CHASE and the CITY harmless from any claims, actions or other proceedings which may be brought against CHASE or the CITY, their agents or employees arising from the CONTRACTOR'S performance of the services as herein stated.
