COLUMBIA HEIGHTS AFTER-SCHOOL ENRICHMENT PROGRAM
CONTRACT FOR SERVICES
IT IS HEREBY AGREED THAT IMMACULATE CONCEPTION SCHOOL (hereinafter called
"CONTRACTOR") will furnish services as herein stated for the COLUMBIA HEIGHTS AFTER-
SCHOOL ENRICHMENT PROGRAM (hereinafter called "CHASE").
This agreement shall take effect upon its execution by both parties and shall remain in effect until
January 15, 2001 at which time it shall be automatically terminated. This agreement is subject to termination
by either party by notice in writing to the other.
SERVICES TO BE PERFORMED UNDER TERMS OF THIS AGREEMENT
Provide the Immaculate Conception School Intramural program for youth in grades 3 -8, which will meet
every Monday and Thursday for 1 ½ hours each day from October 1, 2001 through December 22, 2001.
The Immaculate Conception School Intramural program will be located at Immaculate Conception
School. The CONTRACTOR shall provide all instructors, supplies and equipment, as well as transportation to
and from the program. CHASE shall provide publicity and process the registrations for the Immaculate
Conception School Intramural program, with the provision that Immaculate Conception School
Intramural program hand recruits participants. All other services to be performed by CHASE and the
CONTRACTOR in connection with the Immaculate Conception School Intramural program are specified
in the attached CHASE program letter of understanding, and the mini-grant proposal submitted by the
CONTRACTOR, which are incorporated into this document by reference.
CONSIDERATION TO BE PAID FOR SERVICES SHALL BE:
The CONTRACTOR shall be paid a maximum of $1,650.00 for the provision of the Immaculate
Conception School Intramural program. The CITY OF COLUMBIA HEIGHTS (hereinafter called
"CITY") shall act as the fiscal agent for the disbursement of CHASE funds. Payment shall be issued by check
approximately (3) three weeks after the completion of the program.
AGREED THIS 11th DAY OF September, 2001.
It is agreed that said services are to be rendered as an independent contractor and not as an employee of
CHASE or the CITY. The CONTRACTOR understands that the performance of services as herein stated may
involve some risk of injury or illness to the CONTRACTOR and to the participants of the Immaculate
Conception School Intramural program. CHASE and the CITY will not be responsible for any injury or
illness suffered by the CONTRACTOR, the CONTRACTOR's employees, the CONTRACTOR'S agents or the
participants of the Immaculate Conception School Intramural program during the time the
CONTRACTOR performs the services herein stated. Furthermore, the CONTRACTOR agrees to defend,
indemnify and hold CHASE and the CITY harmless from any claims, actions or other proceedings xvhich may
be brought against CHASE or the CITY, their agents or ej;nployees arising from the CONTRACTOR'S

CONTRACT FOR SERVICES

will furnish services as herein stated for the COLUMBIA HEIGHTS AFTER-SCHOOL ENRICHlVlENT
PROGRAM (hereinafter called "CHASE").
This agreement shall take effect upon its execution by both parties and shall remain in effect until
December 22, 2001 at which time it shall be automatically terminated. This agreement is subject to
termination by either party by notice in writing to the other.
SERVICES TO BE PERFORMED UNDER TERMS OF THIS AGREEMENT
Provide the Y'S Start program for youth in grades 6-8, which will meet twice a week for nine weeks, from
September 24, 2001 through December 22, 2001. The Y'S Start program will be located at Central
Middle School. The CONTRACTOR shall pr ovide all instructors, supplies and equipment, as well as
transportation to and from the program. CHASE shall provide publicity and process the registrations for the
Y'S Start program, with the provision that Y'S Start program hand recruits participants. All other
services to be performed by CHASE and the CONTRACTOR in connection with the Y'S Start program are
specified in the attached CHASE program letter of understanding, and the mini-grant proposal submitted by
the CONTRACTOR, which are incorporated into this document by reference.
CONSIDERATION TO BE PAID FOR SERVICES SHALL BE:
The CONTRACTOR shall be paid a maximum of $1,900.00 for the provision of the Y'S Start program.
The CITY OF COLUMBIA HEIGHTS (hereinafter called "CITY") shall act as the fiscal agent for the
disbursement of CHASE funds. Payment shall be issued by check approximately (3) three weeks after the
completion of the program.

It is agreed that said services are to be rendered as an independent contractor and not as an employee of
CHASE or the CITY. The CONTRACTOR understands that the performance of services as herein stated may
involve some risk of injury or illness to the CONTRACTOR and to the participants of the Y'S Start program.
CHASE and the CITY will not be responsible for any injury or illness suffered by the CONTRACTOR, the
CONTRACTOR's employees, the CONTRACTOR'S agents or the participants of the Y'S Start program
during the time the CONTRACTOR performs the services herein stated. Furthermore, the CONTRACTOR
agrees to defend, indemnify and hold CHASE and the CITY harmless from any claims, actions or other
proceedings which may be brought against CHASE or the CITY, their agents or employees arising from the
CONTRACTOR'S performance of the services as herein stated.

SCHOOL ENRICHMENT PROGRAM (hereinafter called "CHASE").
This agreement shall take effect upon its execution by both parties and shall remain in effect until
December 21, 2001 at which time it shall be automatically terminated. This agreement is subject to
termination by either party by notice in writing to the other.
SERVICES TO BE PERFORMED UNDER TERMS OF THIS AGREEMENT
Provide the Windsock East After-school program for youth in grades 3 -5, which will meet every
Thursday for 1 ½ hours each day from October 11, 2001 through December 21, 2001. The Windsock
East After-school program will be located at Community United Methodist Church. The
CONTRACTOR shall provide all instructors, supplies and equipment, as well as transportation to and from the
program. CHASE shall provide publicity and process the registrations for the Windsock East Alter-school
program, with the provision that Windsock East After-school program hand recruits participants. All other
services to be performed by CHASE and the CONTRACTOR in connection ~vith the Windsock East After-
school program are specified in the attached CHASE program letter of understanding, and the mini-grant
proposal submitted by the CONTRACTOR, which are incorporated into this document by reference.
CONSIDERATION TO BE PAID FOR SERVICES SHALL BE:
The CONTRACTOR shall be paid a maximum of $1,700.00 for the provision of the Windsock East After-
school program. The CITY OF COLUMBIA HEIGHTS (hereinafter called "CITY") shall act as the fiscal
agent for the disbursement of CHASE funds. Payment shall be issued by check approximately (3) three weeks
after the completion of the program.

harmless from any claims, actions or other proceedings which may be brought against CHASE or the CITY,
their agents or employees arising from the CONTRACTOR'S performance of the services as herein stated.

DEPARTMENT (hereinafter called "CONTRACTOR") will furnish services as herein stated for the

This agreement shall take effect upon its execution by both parties and shall remain in effect until

Provide the Middle School Tutoring program for youth in grades 6-8th will be held from September 17,
200! through,December 2!, 2001. The 5~liddle Scho~ol T:ttoring program witl be located at Central
Middle School. The CONTRACTOR shall provide all instructors, supplies and equipment for the program.
The CONTRACTOR shall provide publicity and process the registrations for the Middle School Tutoring
program. All other services to be performed by CHASE and the CONTRACTOR in connection with the
Middle School Tutoring program are specified in the attached CHASE program letter of understanding, and
the mini-grant proposal submitted by the CONTRACTOR, which are incorporated into this document by
reference.
CONSIDERATION TO BE PAID FOR SERVICES SHALL BE:
The CONTRACTOR shall be paid a maximum of $1,675.00 for the provision of the Middle School
Tutoring program (See attachment for breakdown distribution). The CITY OF COLUMBIA HEIGHTS
(hereinafter called "CITY") shall act as the fiscal agent for the disbursement of CHASE funds. Payment shall
be issued by check approximately (3) three weeks after the completion of the program or if other arrangements
have been made for disbursement.

performed by CHASE and the CONTRACTOR in connection with the Valley View Elementary School
Tutoring Program are specified in the attached CHASE program letter of understanding, and the mini-grant
proposal submitted by the CONTRACTOR, which are incorporated into this document by reference.
